COVID-19 Unsafe Worksite Report

Unsafe workplaces during the COVID-19 pandemic put customers, patients, workers' families and every Coloradan at risk. There are over 67.8 million confirmed COVID-19 cases worldwide, 15 million in the US and over 267,000 in Colorado. 14,904 Coloradans have been hospitalized and 3,455 Coloradans have died from COVID-19. As the Colorado economy reopens, every employer's top priority should be protecting the health and safety of employees.

The Colorado Department of Public Health and Environment (CDPHE) calls upon worksites to follow extra guidelines to keep you safe. Read CDPHE's COVID-19 Worksite Safety Guidelines page for more information.

Unfortunately, many employers are not following these guidelines. If you are working in an unsafe worksite we want to know. Please fill out the following form.

Your personal information will only be shared with our COVID-19 Safety Team. Unsafe worksites identified will be shared with Governor Polis, Colorado Department of Public Health (CDPHE) and Colorado Department of Labor and Employment (CDLE).
